Augmentation of alveolar ridge using the modified tunnel technique: a case report

Abstract
Several methods have been devised and used to increase the insufficient alveolar bone width and height, such as guided bone regeneration, alveolar ridge splitting, distraction osteogenesis, and maxillary sinus elevation. These techniques are difficult to perform unless a surgeon is highly skilled. Tunnel technique is a minimally invasive, relatively easy procedure that creates a space between soft tissue and bone by approaching the mesial part of the alveolar bone defect with a single vertical incision and applies bone graft material in it. In the present study, in order to simplify the conventional technique, the modified tunnel technique with the change of incision was introduced and applied on a case of severely atrophied alveolar bone in the maxillary molar area. A 67-year-old female patient was subjected to #27 tooth extraction. 8 weeks later, alveolar ridge augmentation was performed using the modified tunnel technique. After 24 weeks, implant placement and maxillary sinus elevation using a lateral approach were performed. At the time of the implant placement, it was confirmed by opening the bone grafted site through mucoperiosteal flap that the bone graft material was solidly ossified. The preoperative CT showed that the alveolar bone was increased vertically and horizontally, enough for implant placement.
